Please submit supporting documentations according to your account type.

This checklist is not exhaustive; we reserve the right to ask for clarification of documentation provided and/or ask for further documentation.

FOR INDIVIDUAL AND JOINT ACCOUNT HOLDERS

1. Proof of Identity (Must include at least one of the following):
   • Driver Licence
   • Passport
   • Government Issued Photo ID
2. Proof of Residence (proof of residence must dated within 6 months for the address indicated on the application. PO Boxes are not acceptable.):
   • Utility Bill (Gas, Phone, etc.)
   • Current Lease agreement
   • Bank Statement

FOR CORPORATE ACCOUNTS

Client Documentation List:

1. Proof of Existence

   • Certificate of Incorporation
   • Articles of Organization
   • Partnership Agreement (if applicable)

2. Proof of Shareholders\Partners - i.e.

   • Share Certificates
   • Ownership Register

3. If any shareholder\partner owns 10% or more of the entity then:

   • Corporate Appendix
   • Government Issued Photo Identification (Non Expired)
   • Proof of Residence (Dated in the last 3 Months)
4. Proof of Registered Address of corporate entity
5. List of Directors (full names and nationalities required)

Hedge Funds (Additional Documentation Required):

• Prospectus or Offering Memorandum
• Investment Management Agreement
• Anti-Money Laundering Fund Certificate (to be provided by VERTIFX, LLC)
• Anti-Money Laundering and KYC Procedures

Trust Funds (In lieu of Corporate Documents):

1. Trust Deed
2. Certificate of Registration (where applicable)
3. For all Settlors, Successors and Trustees:
   • Appendix
   • Government Issued Identification (Non Expired)
   • Proof of Residence (Dated in the last 3 Months)
4. List of Beneficiaries
   • Names of Individuals
   • Government Issued Identification (Non Expired)
